BACK TO SCHOOL PHYSICALS

If your child will need to have their annual physical exam done before the start of school, we have some openings this week and next. Either call the office or use MyChart to check availability.

Depending upon your insurance, physicals can be scheduled once a calendar year, or every 365 days. Please check with your insurance to ensure you are following their specific rules when scheduling.

The forms that are completed at annual exams are good for one year, unless specifically noted otherwise on the form. Children’s Hospital has made electronic versions of the standard school forms available for our use within the EPIC system. If you need to have other forms completed, be sure to bring them to the appointment so they can be reviewed and completed.

Remember that we cannot sign forms or orders for services, make referrals, or fill prescriptions for any children who are not up to date on annual exams.

MYCHART FORMS

In order to save time, there are standard forms that are completed at each annual exam visit. These primarily include:

• State-Approved General Health Appraisal for use with Day Cares and Preschools
• CHSAA Sport Form – which is generally accepted by most Douglas County Schools, as well as other counties as well.
• School specific forms can be completed if required by individual schools.
• Other more specific forms are completed as needed and indicated by particular patient situations.

How Long are Forms Good for?:

• Most forms are good for one year and are available for parent download through MyChart any time.
• Children under age 4 may require other age-specific intervals since they are seen more than once a year for wellness exams until age 3.

Where can I find my child’s forms?:

• Forms that are generated during an office visit can be found under “Letters” in MyChart.
• Forms that are submitted and completed can be found under “Documents” in MyChart.
• A printable Immunization record is also included in MyChart – type “immunizations” in the search bar.

HOME PHONE FIELD

Within the EPIC system, the home phone field is considered the primary contact phone number and as such is a required field. When it is blank, it creates an error code to be addressed by office personnel. For this reason, you may see a cell number listed in both the home field and as separate mobile number. In this way, we can meet the system requirements, but also know which cell numbers are on the file.

It is sometimes easier, given the size of the EPIC system, for us to search for your children using a common phone number. This is why we will ask for both parents cell numbers to be included on each child’s record.
