SATURDAY PHYSICAL OPENINGS

We have added additional spots into our summer Saturday physical schedule in August, morning and limited afternoon, to help accommodate the need and get kiddos ready for the upcoming school year.

If your child will need to have their annual physical exam done before the start of school, either call the office or use MyChart to check availability.

Depending upon your insurance, physicals can be scheduled once a calendar year, or every 365 days. Please check with your insurance to ensure you are following their specific rules when scheduling.

The forms that are completed at annual exams are good for one year, unless specifically noted otherwise on the form. Children’s Hospital has made electronic versions of the standard school forms available for our use within the EPIC system. If you need to have other forms completed, be sure to bring them to the appointment so they can be reviewed and completed.

Remember that we cannot sign forms or orders for services, make referrals, or fill prescriptions for any children who are not up to date on annual exams.

IMMUNIZATION RECORDS

Your child(ren)’s vaccine record is available in MyChart and can be printed or downloaded for your information and use. Vaccine records do not require a signature unless the printed version has been modified in any way.

If a vaccine record is needed with the Colorado Seal, this can be downloaded from the Colorado Immunization Information System (CIIS) using the public portal. A link is provided in the left side bar.

Please be advised that we do not manually complete vaccine records since the information is the same and available in both systems. The only exceptions are as follows:

• College students who are attending an out-of-state college/university that requires a specific form be completed.
• A vaccine record that is needed to obtain or replace a social security card (only up to age 4).
