The summer wellness exam appointments are filling quickly. If your child is due for their annual physical exam during June, July, or August, contact the office, or use MyChart, to secure an appointment time that is convenient for your family.

Remember that we cannot sign forms, make referrals, or fill prescriptions for any children who are not up to date on annual exams.

We are in peak form request season. In order to decrease processing delays, please remember the following guidelines:

• For standard General Health Appraisal and CHSAA forms, these are completed at each annual wellness exam and available in MyChart. These forms are good for one year.
• If a specific form is needed for a summer camp, please provide the form to us via MyChart or email to fax@parkerpediatrics.com. Please be sure to complete all parent portions prior to submitting. Blank forms will be returned to be revised with required information.Any form that is submitted for processing should be in PDF format. We cannot complete forms from images.

• Immunizations records are downloadable from MyChart. These forms do not require a signature, unless amended or changed in any way.If a form is needed with the Colorado Seal, parents downloaded from the Colorado Immunization Information System (CIIS) using the public portal. There is a link to this site in the left sidebar of this email.

With families spending so much time outside now that the summer season is in full swing, we wanted to reiterate the need for wearing helmets when on bikes, skates, boards, or scooters. Also, don’t forget to apply and reapply sunscreen frequently when enjoying the sunshine, even on cloudy days.
