Graduation Congratulations

To all our graduating seniors and their families we send our sincerest congratulations and best wishes for all their upcoming adventures!

Summer is just around the corner…

Beat the summer rush! With summer sports getting ready to take off, make sure your child is up to date on their annual physical exams. This is a requirement for us to be able to complete any forms, orders, or medication refills.

We have sent out email reminders for any patients that were seen in May and June prior to our conversion to EPIC. Those patients who were seen after that were also emailed a reminder to schedule with the date of the previous year’s physical.

If your child will be due for their annual exam this summer:

Call the office to schedule an appointment
Utilize the MyChart App to check schedule availability and schedule using the app.

Form Requests

Summer and fall are always peak times for form requests.

Standard school forms are being prepared at the annual exams, uploaded to MyChart, and are good for one year – this includes school sports and day care forms.

Camp or other activity-specific forms can be emailed in PDF form, or submitted using MyChart, with all parent portions completed before submitting. Before submitting forms, please be sure the child’s name and date of birth are on the forms. Once completed these will be made available in MyChart.

Immunizations are available anytime through MyChart. The printable record can be downloaded and be provided as needed by parents. This form does not require a signature because it is a printout from the Colorado Immunization and Information System (CIIS).
